There’s a lot of incorrect information floating around the web about how to unlock the secret ending of Cyberpunk 2077, due to an official guide which went to print with outdated information.
You do not need to complete the Kerry Eurodyne/Samurai quests, or get your relationship with Johnny Silverhand to 70%.
Unlocking the secret ending is done by choosing a specific set of dialogue choices in the Chippin’ In mission, when you visit Johnny Silverhand’s grave.
Enemies in the (Don’t Fear) The Reaper mission will one-shot you at level 20, and dying triggers a sad ending, so our recommendation is that you choose the right dialogue during Chippin’ In, then save this ending for once you’re through a lot of the open world activities.

Choose “Think you and Rogue should go.”
Do not take the pseudoendotrizine – wait for about five minutes.
Johnny will say this:
Kinda tough deciding which of your friends get to die, isn’t it? Good news is you got this one chum who’s already dead. And he’d be honoured to join you on a wild, suicide run. You, me and Arasaka Tower. Kinda sounds like a Eurodyne lyric, I know, but trust me – we’ll go fuckin’ nova.
Choose “Let’s hear this plan”.
“Plan?” Well, might be how Rogue operates. I say you grab the hottest iron you can find, stride in the Tower’s front door and cut your own path down to the lower levels. If Mikoshi’s deep underground like Hanako claims it is, you’ll just have to find the elevator.
Choose “[Attack Arasaka Tower] Means goin’ out with a bang – I like it”
This will drop you into the lobby of Arasaka Tower – good luck!
